Welcome to the Patient and Visitor Guide of Emory Crawford Long Hospital Radiology.  We hope you find this information valuable.

Pre-registration for a procedure can be handled in the three different ways listed below:

1. Print and Fax
Print and complete the pre-registration form and fax it to the admissions department at 404-686-8956.  This is the fastest way to pre-register.

2. Print and bring with you to your appointment
Print and complete the pre-registration form and bring it with you to your appointment.  When you arrive, stop by admissions and they can pre-register you at that time. 

3. Complete form the day of your appointment
Do nothing and admissions will try to contact you to pre-register over the phone.  If they are not able to reach you, then you will need to stop by the admissions department the day of your appointment to register.  You should allow 20 minutes for this process.

Directions if you take MARTA:
Emory Crawford Long Hospital is located on the North/South Marta rail line.  The hospital is located between the Civic Center and North Avenue stations.  You may use either station.  Please visit the MARTA Web site for specific train schedules. 

Parking:
Visitors and patients may elect to use either the valet parking service located in front of the hospital, or the self-parking deck, which can be accessed off of Peachtree Street at the new hospital drive.   

Directions from the visitor's parking deck to the department: 
After you are parked, locate the signs that say, "To Hospital and Medical Office Tower."  They are located on the East side of the deck on each level.  This is the access corridor to the skywalk, which is located on Level F.  From deck level F, exit left off of the elevator and cross the skywalk to the second floor of the conservatory.  Take the escalator down to the lobby level.  As you go down the escalator, look to the left and you will see the hospital elevators.  These are the elevators that you will use to go to radiology, which is located on the ground level. 

Directions from valet to the department:
As you enter the hospital conservatory, proceed to the hospital elevators located in the back.  Use these elevators to go to radiology on the ground floor.
